Our Policies

Your Appointment

Appointments are reserved just for you. Please arrive 5-10 minutes early. Arriving more than 10 minutes late may result in a shortened or canceled session, and full charges still apply.

Cancellations

We require at least 24 hours’ notice by phone to cancel or reschedule.

  • No fee if notice is given 24+ hours in advance.
  • $50 fee for appointments 45 minutes or less.
  • $150 fee for injectable or longer appointments.

These fees will be charged to your card or must be paid before rebooking.

No-Show Policy

After 2 no-shows or last-minute cancellations, a non-refundable deposit of $50-$150 may be required to book future services.

Card on File

All clients must have a valid credit card on file, stored securely. This card may be charged if our cancellation policy is not followed.

  1. Joining Elite Memberships: Both new and existing patients are eligible to join Elite Rewards. After the registration form is completed, a patient coordinator will assist you with putting a debit or credit card on file and establishing your billing date.
  2. Promotional Rates, Offers & Privileges: As an Elite Reward member, you will receive member-only rates, exclusive offers, and any additional membership privileges are guaranteed so long as you remain in good standing, including but not limited to: monthly payments and paying any remaining balance at your visit. These rates and offers cannot be given, purchased, or transferred to another person who is not a member of Elite Rewards. Purchases made with Elite Rewards and offers cannot be combined with any additional promotions or offers. Rates, offers, and privileges are subject to change at any time without notice. Elite rewards and discounts cannot be applied to any packages or promotions.
  3. Monthly Billing: An active debit or credit card on file and 1 year (365 days) of recurring monthly payments are required. Your recurring monthly payments occur after your initial visit. At your initial visit when you sign up for Elite Rewards, $199 is charged and applied to your total balance that day to get rewards set up in the system. This is not considered your first billing date. Your first monthly membership payment will occur after this payment on the next billing cycle. One year (365 days) of recurring monthly payments are required to keep your membership in good standing and to avoid any cancellation penalties. Your account will be billed every 30 days on either the first or fifteenth of the month (determined by your date of sign up) for $199 per month. By signing, you agree that your payments of $199 will be automatically withdrawn every month every 30 days, these will continue until you cancel your membership (see cancellation terms in section 5). All sales are final and there are NO REFUNDS for any reason. You may not move, stop, or delay payments, by doing so your membership may result in a permanent cancellation and/or penalties. You authorize Elite Medi Spa to charge your card monthly and for any additional services that are rendered. If your available funds in Elite Rewards are not sufficient to cover the cost of the procedure being done, you will be required to pay the remaining balance the day they are performed prior to leaving the office. Your monthly payment will be added to your available funds to use at any time at the Medi Spa. Your funds never expire if you are an active rewards member.
  4. Unsuccessful Payments: In accordance with the applicable law, if the first attempt to collect any fee under this agreement is unsuccessful, we may make additional attempts to collect from any/all payment methods you provide us. We will attempt collection via phone call and/or text message to the phone number on file for 3 business days after initial decline. A $25 service fee will be applied for each instance we submit or re-submit such a payment request after those three days and it is returned uncollectable for any reason including but not limited to: insufficient funds, expired or canceled payments, overdrafts, or closed accounts. We are not liable for any fees charged by your financial institution in the event a payment request is returned uncollectible. Failure to pay your balance will result in back-paying for any payments missed and termination of account. Elite Rewards Card Declines: If your card on file declines, we make multiple attempts to reach you to attempt to get your account in good standing. If your card on file declines multiple times over more than one billing cycle, then a second card on file will be required.
    Elite Rewards Members who do not update their card on file for membership within 30 days of card decline will automatically be canceled and not be able to join Elite Rewards, as existing funds on the account will only be available for 3 months (90 days). If the card declines within the first 3 months of membership, you will be subject to an early cancellation fee of $199 and will not be able to rejoin Elite Rewards in the future. If your card declines within the first 3 months of membership and remains unpaid, the amount for any unpaid invoices OR the discounted amount from prior services may be sent to collections. If your card on file declines, you will not be permitted to schedule any appointments until you have updated your card and payment has been processed.
  5. Cancellations: A One year membership commitment is required to join Elite Rewards. If your cancellation date is within the first 365 days of membership, you will be required to pay a cancellation fee of $199. This cancellation fee will NOT go toward your funds available. A 30-day notice and payment for those remaining 30 days of membership is due to cancel your Elite Rewards membership. Your final payment will be processed when your cancellation request form has been submitted. You may not move, stop, or delay payments, by doing so your membership may result in a permanent cancellation. A written request must be delivered to Elite Medi Spa via electronic form that can be sent through the client portal or filled out in person. Failure to fill out this form will result in your membership cancellation to be delayed and not canceled. Once processed, you will be given your official cancellation date 30 days from the date of processing. There are NO REFUNDS. All sales are final. Your membership is still valid for the remaining 30 days once cancellation has been processed, unless you do not pay your final monthly payment. Failure to pay your last monthly payment will result in not having access to your available funds until your balance is paid, only having 3 months to use your available funds, and not being allowed to rejoin Elite Rewards in the future. The remaining funds paid will be put toward your available funds to use towards products and services. If you choose to cancel, your funds are available to use as a credit for six (6) months after your cancellation date. Once your membership has officially ended, you will no longer have access to members-only rates, offers, and privileges. If you do not use your accumulated funds within 6 months of your cancellation date, you forfeit the balance and will not be able to use those funds in the future. Memberships may be reinstated if the account was in good standing and approved by the spa manager. You may reinstate your membership at any time before the 1 year expiration date to use your previously accumulated funds and to receive your membership only rates, offers, and privileges. If you decide to reinstate your membership, you may be required to pay a registration and processing fee at sign up and your membership will begin as if it is new, on the reinstated date. You may reinstate your membership one (1) time in accordance with the terms.

Refund Policy

  • No refunds on services once purchased.
  • Unused package balances can be applied to other services.
  • All injectable and skincare sales are final.
  • Products may be refunded within 7 days only if a documented adverse reaction occurs.
